Botulinum toxin is often talked about as if it does the same thing for everyone at every age. In reality, the way it’s discussed and the goals behind it change a lot as your face, skin and life change too.
One of the most common things people say to me in clinic is, “I don’t know if this is something I should even be thinking about yet,” or “I feel like I’ve left it too late.”
The truth is, there isn’t a right or wrong age. What matters far more is understanding what’s happening in your face and having a proper, informed conversation about it. As a nurse, my approach is always to assess first - your skin, your facial movement, your structure - and then talk things through calmly and honestly. That’s why everything starts with a consultation, so you feel informed and supported, not rushed or pressured.

If You’re in Your 30s: “I’m Starting to Notice Little Changes”
If you’re in your 30s, you might be noticing small shifts in your face. Lines may appear when you frown or concentrate, but disappear again when your face relaxes. You might just feel like you look more tired by the end of the day than you used to.
At this stage, consultations are very much about understanding how your face works. As a nurse, I look closely at facial movement patterns, muscle strength and early signs of skin change. We talk about what’s normal, what’s age related, and what (if anything) may be worth keeping an eye on as time goes on.
For many women in their 30s the goal isn’t treatment at all, it’s prevention. That’s why education, reassurance and awareness are so important to me for my client’s - so any decisions made later on feel informed and considered rather than reactive.

If You’re in Your 40s: “I Look More Tired Than I Feel”
In your 40s, changes often become more noticeable. Lines may sit a little deeper, and expressions that once felt neutral can start to give the impression that you’re tired, stressed or worried even when you’re not!
This is where a nurse-led assessment really matters. Rather than focusing on one line or area, I assess how the muscles, skin and structure of your face are working together. Conversations here tend to focus on softening stronger expressions and easing heaviness, while still keeping everything looking very natural.
We also talk a lot about skin quality at this stage, because collagen loss, hydration and texture changes play a much bigger role now. Looking at the face as a whole allows for a more balanced, thoughtful approach rather than treating one concern in isolation.

If You’re in Your 50s: “I Still Want to Look Like Me”
By the time you’re in your 50s, facial ageing becomes more complex. Muscle strength, skin laxity and changes in bone structure all influence how the skin sits on the face. That’s why treatment goals naturally shift again.
Many women at this stage tell me they’re not trying to look younger, they just want to look softer, less tense, and more like themselves. As a nurse, my focus here is on gentle support and smoothing, rather than correction or change.
Discussions are usually about reducing tension that pulls features downward and supporting facial balance. Being conservative is key, because overly aggressive muscle relaxation can sometimes take away from natural harmony rather than enhance it. At this stage especially, clinical judgement and anatomical understanding are essential.
